Join National Health Foundation and PFCC Partners at the fourth annual Patient and Family Centered Care (PFCC) Conference. This conference is a unique opportunity to share challenges, solutions and successes related to PFCC with a diverse healthcare network. This year, the conference features two dynamic keynote speakers who will both inspire us and get us thinking in new and innovative ways.   Robert M. Wachter, MD is Professor and Associate Chairman of the Department of Medicine at the University of California, San Francisco, where he holds the Lynne and Marc Benioff Endowed Chair in Hospital Medicine. He is also Chief of the Division of Hospital Medicine, and Chief of the Medical Service at UCSF Medical Center. He has published 250 articles and 6 books in the fields of quality, safety, and health policy. He coined the term “hospitalist” in a 1996 New England Journal of Medicine article and is past‐president of the Society of Hospital Medicine. He is generally considered the academic leader of the hospitalist movement, the fastest growing specialty in the history of modern medicine. In 2012, he was named the 14th most influential physician‐executive in the U.S. by Modern Healthcare magazine, the fifth year in a row in which he was the most highly ranked academic physician on the list. Check out his website for more about Dr. Wachter: http://community.the‐hospitalist.org/   Dave deBronkart is a cancer survivor known as “e‐Patient Dave” and an international keynote speaker on healthcare. Dave is actively engaged in opening health care information directly to patients on an unprecedented level, thus creating a new dynamic in how information is delivered, accessed and used by the patient. This is revolutionizing the relationship between patient and health care providers, which in turn will impact insurance, careers/jobs, quality of life and the distribution of finances across the entire spectrum of health care. His compelling TEDX Talk “Let Patients Help” is in the top half of most‐watched TED talks of all time.
