Use this form as a guide for submission of proposed symposia. The information must be typed, no handwritten submission will be accepted. A biographical sketch for each individual speaker, chair(s) and discussant(s) must be included in the submission.

Scientific Importance: Please outline why this symposium will be of interest to the membership of ASCPT.
Speakers: Please summarize any important information regarding the stature of the speakers in this scientific area and confirm that they have all agreed to speak if this symposium is selected.
For each speaker, please list name, degree, title, institution, address, phone and fax number, and email address. If possible, please include name, phone number, and email of a staff person in the speaker's office for contact.
Objectives: Please summarize the key objectives of the symposium.

Guidelines for 2001 ASCPT Symposia
Please note the changes to the Symposia guidelines. The format for ASCPT's 2001 meeting has changed and Symposia will be offered on Wednesday, March 7. The afternoon sessions on Thursday, March 8 and Friday, March 9 will be devoted to Scientific Section Workshops.
Two hours will be allotted for each symposium.
The goal of each symposium is to provide state of the art information about an important topic that is of interest to a significant portion of the ASCPT membership.
Symposia are usually arranged around two invited nonmember speakers and one or more member speakers.
ASCPT members are not eligible for travel reimbursement or honoraria. Members are expected to register for the annual meeting and pay the member registration fee.
A maximum of $3,000 will be allotted for each symposium. These funds include all expenses for airfare, hotel, food, other travel expenses and honoraria. Expenses (within the $3,000 total budget per symposium) will be reimbursed following the meeting. Oral commitments made to speakers in violation of this Board policy will NOT be honored.
Nonmembers of the Society are eligible for reimbursement using the following criteria. Coach airfare booked through ASCPT's travel agent with tickets booked at least 30 days in advance, but no later than 14 days in advance of the meeting. One or two night's hotel at the ASCPT group rate and food. Receipts must be submitted for all expenses exceeding $10 and expenses must be itemized. Nonmembers are eligible for an honorarium not to exceed $500. However, please note that the total symposia expenses are limited to $3,000. All nonmember speakers are expected to follow the above travel and reimbursement policy.
Upon submission of the symposium information you must assure ASCPT that you have contacted all of your symposium participants.
If your symposium is selected for presentation, ASCPT will notify you as well as sending letters to each of the faculty. All official communications regarding financial/travel/accommodation arrangements will come from the ASCPT office.
The Scientific Program Committee will not consider proposals that are incomplete.
