Program Summary

Strategies to improve chronic care have become imperative to our organizations due to the financial impact of chronic care, resource limitations, increasing patient empowerment, and the demand for measurable performance outcomes. San Diego health care systems each have their own internal quality programs that address chronic care management, yet cross-organizational collaboration has been somewhat limited. Collaboration and cooperation across health care systems has the potential to offer each medial system the opportunity to create more value in chronic care management than any single system standing alone.

This program aims to describe successful office practice management tools that may be applicable to your practice environment and to develop a networking opportunity for individuals and group medical practices interested in sharing and exchanging ideas for best practices in management of chronic illnesses in San Diego.

Overall Course Objectives

After participating in this course, the attendee should be able to:
Understand the concept of design and implementation of Chronic Care Models
Exchange ideas with local leaders and interested parties in successful practice management of chronic illness
Discuss better ideas for solving real world problems through small break out sessions and group interactive discussions
