Description This is the second in a series of international conferences on risk in the delivery of health care organised by the University of Central Lancashire. The first explored the issue of overcoming barriers to adverse incident reporting and as a result of feed-back from participants we have endeavoured to develop a programme which will aim to provide practical guidance in preparation for the introduction of revised healthcare standards in 2007. We will also once again try to learn from other highly regulated organisations and on this occasion lessons from rail and air travel.

Although the delivery of healthcare in the UK is generally safe and well recognised internationally it is inevitable that in complex systems things will and do go wrong during a patient’s journey through the health and social care system. The afternoon session will therefore explore a number of key problem areas from the viewpoints of the patient and carer, the professional and the lawyer.

There is increasing pressure in view of the introduction of the new performance framework Standards for Better Health which will set out the level of quality all organisations providing NHS care will be expected to meet. With plurality of care and patient choice this will inevitably spread across all healthcare sectors. The forthcoming publication of these revised National Minimum Standards from the Healthcare Commission and Department of Health will mean that we all need to be sure that we are ‘up to speed’ in the management of risk.

Conference Aims
This conference will provide the opportunity to hear presentations from nationally and internationally recognised professionals in risk avoidance, clinical governance and safety in public health issues as well as a media figure speaking from the consumer’s perspective. The aim is to broaden the existing network of colleagues from a diverse field who recognise human factors in population safety and have responsibilities to advance the development of high reliability organisations.

At the conclusion there will once again be a session designed to take forward the next stage of our work.
