Continuing Education Accreditation and Designation
This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of the National Association of Managed Care Physicians (NAMCP), and the American Association of Integrated Healthcare Delivery Systems (AAIHDS). NAMCP is accredited by the ACCME to provide continuing medical education to physicians. The NAMCP designates this continuing medical education activity for a maximum of 26.25 credits in Category I credit towards the AMA Physicians Recognition Award. Each physician should claim only credits that he/she actually spent in the educational activity. Application for CME credit has been filed with the American Academy of Family Physicians. Determination of credit is pending.
