Employers, health plans, provider organizations, government & policy leaders are clamoring for changes to existing physician care delivery & compensation structures. A growing shortage of primary care physicians & the growing prevalence of chronic disease has accelerated demand for a new model of care delivery. The Medical Home, as a new model for primary care, has emerged, evolved & gained considerable traction, even being embedded in Medicare pilot programs enabled under the Tax Relief & Health Care Reform Act of 2006.
