Care homes carry out an essential role in supporting people approaching the end of life and their families. One in five people aged over 65 die in care homes in England and Wales and many more residents will have end of life care needs but die in hospital or another setting. The national and regional drives to support people in the community as they approach the end of life and to reduce the numbers of people dying in hospital can only succeed by partnership working with care homes. There are many examples of good end of life care within care homes, both residential and nursing. This conference will provide an opportunity both to learn about some of the national work to support care homes and to examine in more detail approaches taken by care homes and partner organisations working with people approaching the end of life.
