Roundtable sessions examining the imperative role of Case Managers in coordinating client transfers between differentiated, multi-level systems and achieving a cohesive client experience in a range of care sectors. The sessions and discussion groups at the conference will be used to facilitate the production of a competency document for practitioners reflecting the knowledge, skills, abilities, attitudes and judgment necessary to practice safely and effectively.
