“Too many patients don’t get the seamless effective service they should when they leave hospital. In fact we’ve seen a 50 per cent increase in the number of emergency readmissions in the ten years from 1998. We need to do more to prevent this from happening.”
Rt Hon Andrew Lansley CBE MP, Secretary of State for Health (October 2010)
Around 35,000 people will benefit from a £70 million cash boost that will enable the NHS to support people back into their homes after a stay in hospital. Capita’s National Reducing Hospital Readmissions Conference brings together representatives from the NHS, local authorities and voluntary organisations to share best practice and identify strategies for using this extra funding to ensure local people get a seamless service after discharge.

Keynote presentations and practical case studies examine how PCTs can work more closely with hospitals and local authorities in the delivery of post-discharge services. Benefit from best practice on ensuring access to high quality care, driving down readmissions and achieving efficiency savings across health and social care provision.
